NHSN Daily Facility Operating Status (DFOS)

NHSN is committed to patient safety and ensuring the readiness of healthcare facilities across the United States. In response to the evolving threats to healthcare infrastructure, the Daily Facility Operating Status (DFOS) module was developed as a tool for healthcare facilities to report and monitor the status of critical infrastructure. These standardized essential elements of information enable ongoing monitoring of operations and allow stakeholders at the federal, state, and local levels to effectively track facility readiness and support resilience-building efforts.

The DFOS module is available for optional reporting of critical infrastructure information for acute care or other short-term stay hospitals. Please note that reporting data using this form does not replace any regulatory reporting requirements.
